Footnotes:

(1) These are the dates forms are due IN the University Graduate School. Submit forms to the Academic units at an earlier date to allow sufficient time for approval and signature. Ultimately, it is the student's responsibility to make sure forms are received by the University Graduate School on time.

(2) FORM D-1 should be submitted by doctoral students as soon as the dissertation committee has been formed. FORM D-1 should typically be submitted one semester before FORM D-3 but no later than the deadline: FOUR semesters before the anticipated graduation term.

(3) FORM D-3 should be submitted by doctoral students as soon as the dissertation proposal has been developed and approved by the dissertation committee. This should be early in the dissertation process to allow maximum dissertation committee input. Form D-3 should typically be submitted within a semester after FORM D-2 but no later than the deadline: THREE semesters before the anticipated graduation term.

(4) FORM M-1 should be submitted by master's students as soon as the thesis committee has been formed. FORM M-1 should typically be submitted one semester before FORM M-2 but no later than the deadline: TWO semesters before the anticipated graduation term.

(5) FORM M-2 should be submitted by master's students no later than the deadline: ONE semester before the anticipated graduation term.

(6) FORMS D-5 (doctoral students) and M-3 (master's students) must be submitted to the University Graduate School at least 3 weeks before the date of the defense or by the deadline (whichever date is the earlier).

Important information:

(a) Doctoral students: Submit FORM D-2 as soon as candidacy examination's results are available. Students will be allowed to register for dissertation credits AFTER submitting FORM D-2 to the University Graduate School.

(b) FORMS D-4 should be submitted by doctoral students to the University Graduate School for the first time within a year after FORM D-3 and at least yearly thereafter until the dissertation is completed.

(c) FORMS D-6 and M-4 should be submitted to the University Graduate School no later than 2 weeks after the date of defense.
